The Joke

She can't quit because she has a genuine, severe drug addiction. The final panels show that spiders are everywhere because they are "attracted to her morning coffee" (i.e., the drugs), and the woman concludes it's still worth it, maintaining her blissful ignorance about the severity of her actual situation.
The Humor
The comic satirizes the way people casually claim to be "addicted" to coffee, treating mild caffeine dependence as though it were a serious condition. By having someone actually replace the coffee with genuinely addictive and dangerous drugs, the comic takes the exaggerated self-diagnosis to its logical extreme.
