Explanation
This comic is a parody of a romantic "say my name" moment between a couple. The man asks his partner to say his name, and she responds with "Oh Nyarlathotep, or Chaugnar, the dread spawn of Cthulhu" -- names of Lovecraftian elder gods and cosmic horrors from H.P. Lovecraft's Cthulhu Mythos.
In the final panel, the man (whose name is actually John) is lying in bed with his partner, who asks "You still think about him, don't you?" -- implying that the woman once dated or was involved with a Lovecraftian cosmic horror, and her Freudian slip during an intimate moment reveals she hasn't fully moved on. The joke works on multiple levels: the absurdity of a woman having an ex-boyfriend who is an eldritch abomination, and the very relatable human experience of accidentally calling a current partner by an ex's name.
